  • Gloucestershire Cruse Bereavement Care offers one-to-one counselling sessions, therapeutic groups and friendship groups to people trying to come to terms with the death of others. Tel: 01242 252518 or 0808 808 1677 (National help-line)​
  • Longfield Day Hospice offer bereavement support groups and individual support for adults in Gloucestershire who have been bereaved by a death from illness or other natural causes. Tel: 01453 886868
  • Great Oaks bereavement Service is mainly for the family and friends of the people who have used their services Tel: 01594 811910
  • Sue Ryder website has leaflets on bereavement, online counselling and support groups.

Report of Death to the Coroner - Electronic Form

Please follow the resource link below to the Electronic Referral document for GPs to refer deaths for PM or 100a.

This is a generic form for use by both primary and secondary care. The form is to be used for all death referral where no advice is needed. Referring GPs / Practice can expect to receive by way of a response from us:

  1. Referral received and accepted for further consideration by the coroner. (i.e. in the case of referral for PM/inquest)
  2. Referral received and 100A issued.
  3. Referral received and further discussion required. In which expect a follow up call from the HMCO.
