Falls Assessment and Education Service (FAES) - Clinical Specialist Physiotherapist

The Falls Assessment and Education Service (FAES) consists of five clinical specialist Physiotherapists, one based in each locality throughout the county. The Physiotherapists provide multifactorial specialist falls assessment with a focus on mobility, physical activity, balance and strength. This may be an outpatient appointment or may take place in the place of residence. For patients who:

  • have had two or more falls in the last 6 months

Prior to referral please ensure that a medication review has been completed

Medical Falls Assessment Clinic (FAC)

To be referred if

  • Recurrent (2+) falls in the past 6 months.
  • Aged 65 or over.
  • Needs further medical investigation (e.g. dizziness/blackouts/vertigo)
  • Complex medication issues

Consultant/medical led multi-disciplinary Falls Assessment Clinics (FAC) currently operate at Gloucestershire Royal, Cheltenham General, Tewkesbury and Dilke hospitals. These clinics provide specialist multi-disciplinary (MDT), multifactorial assessment and intervention to identify and reduce the risk factors for falls.

Suspected syncope should be referred directly to the specialist Syncope Clinic at GRH.

There is insufficient evidence to recommend interventions to prevent falls in older persons with known dementia, however we will see these patients if you feel they could benefit. 

  • Medical examination
  • Medication review and advice on de-prescribing
  • Vestibular assessment and therapy
  • Bone health assessment
  • Vision assessment
  • Height and weight measurement
  • Lying and Standing blood pressure measurement
  • Mobility, strength, gait and balance assessment with provision of walking aids as necessary
  • Assessment of activities of daily living
  • Relevant investigations such as an electrocardiogram (ECG), X-ray or blood tests 

Patients may be required to return to a follow-up clinic or have further tests. Patients may be taught exercises to improve their strength and balance, be referred to an active balance class or other relevant exercise programme. Patients will be given advice or equipment to help increase their physical activity.

The Doctor will write to the GP and the person who made the referral (if different), with a summary of the risk factors identified for falling and any recommendations to reduce the risk. 

Please include information on:

  • The history of the most recent fall
  • A history of other falls in the past 12 months
  • Past medical history
  • Medication review & current medication
  • Social history
  • Recent test results:
    • Electrocardiogram (ECG)
    • Lying and standing blood pressure
    • Routine bloods (additional tests where clinically indicated)
    • If no bloods within past three months, please arrange prior to appointment
