Local Diabetes Education Sessions

Diabetes and You

This is a three-hour session led by a diabetes educator. It is intended for anyone with Type 2 Diabetes or carers, regardless of length of diagnosis. During the session patients will have the opportunity to meet with other people who have Type 2 Diabetes and ask questions.
 

Diabetes, Food and You

This is a two and a half hour session led by a Registered Nutritionist or Dietitian. It is intended for anyone with Type 2 Diabetes or carers, regardless of length of diagnosis.
 

Insulin and You (Education for Diabetes and Insulin Therapy)

This is an exciting patient education group aimed at people who are new to insulin. The sessions are run in small groups using Conversation Maps. Conversation Maps are a new way to learn about diabetes and insulin through group activities facilitated by a skilled diabetes healthcare professional. Follow the link for more information.

All of these sessions are self-referral and if a patient would like to attend they need to fill in the referral form within the information leaflet here and return it to the community diabetes team.

Mapmydiabetes - Online Support Tool - Gloucester and Forest of Dean localities only

Mapmydiabetes is a online support tool for people with Type 2 diabetes available to practices/patients in Gloucester City and the Forest of Dean in the first instance (September 2016 – March 2017).

It can be used on a computer laptop tablet or smart phone. It is offered through primary care which creates accounts for registered patients. It provides patients with structured education and validated learning resources about diabetes with the aim of supporting them to self-manage. The tool includes the capacity for patients to record personalised information and set targets in collaboration with their nurse/GP.  There is no facility to refer new patients at this present time.
