Gloucestershire’s new healthy lifestyles service is an integrated service that will provide people with lifestyle support through one single point of access, rather than through separate services.
Stop smoking, weight management, alcohol reduction and physical activity support, previously provided separately, will now be provided through this one integrated service.
The Healthy Lifestyles Service can support people who would like to increase their physical activity, but do not require specialised support to do so (or Patients for who physical activity is considered a contraindication) . Individuals who require more specialised services (such as Cardiac Rehabilitation and Falls Prevention) should be referred to the Exercise on Referral pathway (see Exercise on Referral section below).
Please be aware that the Gloucestershire Healthy Lifestyle Service is a countywide service that offers tier 2 support for people who are ready to make a positive lifestyle behaviour change. It does not replace other locality based lifestyle services within the county which may offer tier 3 support (e.g. Exercise on Prescription schemes or cardiac and respiratory rehabilitation classes)

How to make a referral
Referral into the Healthy Lifestyles Service can be made electronically or via telephone on the contact details below.
Please note: Healthy Lifestyles Gloucestershire Service will attempt to contact a patient within two working days of receiving the referral.
Exercise on referral (EOR) supports people with medical conditions (who are not normally active) to access a supported exercise programme (normally for 12 weeks) with the help of a specialist adviser. The majority of these take place in local leisure centres and community venues.
