G-Care aims to be the principal online point of clinical reference for Gloucestershire’s clinicians.
It is designed to improve the consistency of clinical care delivered across the Gloucestershire Integrated Care System by promoting locally agreed clinical advice, pathways of care and resources for individual clinical conditions.
G-care content has been developed in collaboration with local clinicians and specialists, to support GPs to provide their patients with the care they need, in the right place with minimal delays.
On this site you will find a range of useful resources, including local care pathways, clinical guidance, referral forms, service information and links to community resources.
This site is managed by the CCG’s Planned Care Team and will continuously evolve through the development of clinical guidance and the user feedback received.
The information on this site is guidance and clinicians will still need to use their own clinical judgement to determine the appropriate care for their patients.
This site is intended to support clinicians to deliver patient care, rather than provide a patient information service. If you are not a clinician we would recommend exploring the NHS Choices website as an alternative.
